How sure are we that a NGC # begins with a "3" is an indication that it been to NCS first?

Cert numbers that start with 1 are NGC or NCS, it depends on when the coin was processed and what special services were requested such as multi-holders, large holders, etc.
Cert numbers that start with 2 are NGC only.  These submissions are done via online form or done in China or Hong Kong.
Cert numbers that start with 3 are NGC or NCS submissions.  For NGC they are done with paper forms for NCS they done either online form or paper form.
Cert numbers that start with 4 are NGC or NCS submission.

If you send coins to NCS using the modern $26.50 tier there is no way to know a coin has been conserved.  Only when you use the non-modern tier in which they charge 5% conservation fee do you know some attempt at conservation was made.  If no conservation is made then the fee is 1%.

There are other exceptions to the above rules, for example you can submit coins via online NGC form with cert number starting with a "2" and out of 50 coins they may recommend 5 coins could benefit from conservation, those five coins would be returned with starting serial number of "2". 

Bottom-line there is no way to know if a coin has been conserved, unless you sent it in and can notice a before/after difference.
